Aside from differences in icon sizes, these two settings will not bring any other changes in the way that you interact with your Home Screen pages and app icons on the iPad. Tapping this takes you further in to that particular shortcuts settings, where you can rename your shortcut (usually to be the same as your apps name, or you can make it something unique to you) and select an image for your apps icon. Whenever you use your new app icon, there will be a banner on top of the screen, notifying the launch of the shortcut you created.
Make sure to click Open Appsa with a multicolored icon, so that your first is not already attached to any particular app. Go back to Shortcuts and tap the ellipsis of your created app launcher shortcut to add another custom icon to your lock screen for this app.
